Tomi Ogundimu, Practice Manager, Population Health, Advisory Board

Join MEDITECH and the Advisory Board Company for this webinar on social determinants of health.

Tomi Ogundimu is the practice manager of population health for the Advisory Board. In this capacity, she works with population health and care transformation leaders on a wide range of projects, including developing an integrated care management model, measuring the ROI of their interventions, care delivery innovation, post-acute care network development, and patient engagement.

Ms. Ogundimu understands that in order to implement successful population health strategies, an organization needs a team with three things: diversity in healthcare experience, skills outside of healthcare, and ability to get things done without leaning solely on tech executives.

In her work with population health leaders, it’s clear that such providers know that health care delivery is incomplete without addressing the social determinants of health. But effective patient management cannot only include tasking care teams with addressing patients' social needs on top of their complex clinical needs. Instead, providers should also partner with community-based organizations already providing quality non-clinical support for a range of needs, from healthy food access to stable housing, to scale patient management beyond traditional care settings. Ms. Ogundimu’s session will take all that into account as she discusses the role of community partnerships to advance population health management.
